Claim History & Proof of Professional Liability (Malpractice) Insurance Coverage Requests

When you are credentialed at our institution or any other institution, the credentialing departments will request information about any claims or actions against you (claim history / loss run report) as part of your credentialing process. With external requests they will also require proof of your medical malpractice or Professional Liability insurance coverage. With your written authorization UCSF Health Risk Management will research your UCSF claim history and respond with an original letter to the institution stating that you are / were covered by UC Regent’s Self-Insured Program and provide them with your claim history too.

Please be aware of these three points:

  1. Please note that a COI in the University of California world means Professional Liability insurance coverage for a UCSF provider if they are going to work at a non-UCSF location as a UCSF employee. If the provider is applying for a new job at a non-UCSF location and will no longer be a UCSF employee once accepting that new job, then this is the correct page to be on. The COI and the letter are two very different things in the UC world unlike other institutions where the COI is one and the same for them. Please make sure you are requesting the correct one. If an institution will be reaching out to UCSF Health Risk Management on your behalf for claim history and proof of Professional Liability insurance coverage, please refer them to our public website for details including contact information of our four Risk Management offices broken down by which campus(es) the provider worked at under #10 on this web page:  https://healthriskmanagement.ucsf.edu/ucsf-claim-history
  3. We do not run claim history reports for personal interest. If you have never been served with a claim or if the claim you were served has been closed for over six years, you will have a negative UCSF claim history (please see letter G below for more information). Please note that some institutions may request that your claim history go back further than the six years that UCSF follows; please let us know this on the PDF form under letter A-1 below as requested.

A) How to request your claim history and proof of PL insurance coverage letter from UCSF:
  1. A claim history requires your signed authorization to permit release of this confidential information. Please click on the link to open and print the PDF form and please fill in the required information on it. If it is not properly filled out, it will be returned to you for revisions: UCSF Health Authorization to Release Claims Hx or PL Verification PDF form
  2. Email the completed form to the appropriate UCSF Health Risk Management office location(s) that you worked at or attended:    

a) For UCSF Health San Francisco locations (Parnassus, Mission Bay, Mount Zion, UCSF Health Stanyan Hospital, UCSF Health Hyde Hospital, Zuckerberg San Francisco General Hospital [ZSFG]) please send them to [email protected]. Please note that UCSF Health Stanyan Hospital and UCSF Health Hyde Hospital are community hospitals so please make sure the provider is/was a UCSF employee during their time at those locations.

b) For UCSF Benioff Children's Hospital Oakland for UCSF employees please send them to [email protected]for non-UCSF employees please send them to m[email protected]. Please note that UCSF BCH OAK is a community hospital so there will be UCSF and non-UCSF employees working there.

c) For UCSF Fresno, please send them to [email protected]

d) For UCSF @ Zuckerberg San Francisco General Hospital (ZSFG), please send them to [email protected]

    3. Claim history responses may take up to 10 business days or less upon receipt of request.

C) What are the coverage dates?

Coverage is concurrent with dates of employment/attendance from the first day to the last day at UCSF. If you need these dates, please click on this link: Employee & Student Verification and follow the instructions. UCSF Health Risk Management does not provide these dates.

D) Is there a policy number, face sheet, or certificate for the UCSF PL insurance?
  • There is no policy number, face sheet, or certificate for coverage. The reason for this is that UC Regents owns their own medical insurance under the California Tort Claims Act, section 810. For more information, please click on this link: UCOP BUS 81
  • A letter verifying coverage may be requested by contacting the UCSF Risk Management campus you attended/worked at, please see #2 above.
  • If you are filling out an online application, you can enter in "00000000" or "N/A" for the policy number section.
E) What is the coverage amount?

Coverage always meets the industry standard, currently $1,000,000 per occurrence; $3,000,000 aggregate.

F) Is tail coverage provided? Is my PL coverage claims based or occurrence based?

UCSF's insurance coverage is occurrence based, so there is no need for tail coverage. Occurrence based policy: the incident must have happened while the policy was active. It offers lifetime coverage for incidents that occur during the policy period regardless of when the claim is reported. 

A claims-based policy only covers incidents that occur and are reported within the policy's time frame; therefore, a tail extension coverage would be needed.

G) How far back does a claim history report go?

Risk Management will report claims (notices of intent [NOI] to sue or demands for compensation) or litigation (lawsuit) arising from alleged professional negligence in which the provider was either a named party or in which the provider's care was the subject of the claim and reported to us within the past six years. Six years is the standard reporting time frame per UCOP guidelines. Depending on where you are applying for a job you may need it to go back further (10 years) or for the entirety of your time at UCSF.
