Certificate of Insurance (COI)

There may be times when a provider will work at a non-UCSF owned healthcare facility. That facility will request a Certificate Of Insurance (COI) verifying Professional Liability insurance coverage (PL / malpractice insurance) for this provider. If the provider's Department Chair is aware of these activities and approves them as being within the course and scope of their UCSF employment, then a COI will be issued for the provider to work at that non-UCSF facility. 

Please be aware of these three points:
  1. Please note that a COI in the University of California world means Professional Liability insurance coverage for a UCSF employee if they are going to work at a non-UCSF location. If the provider is applying for a new job at a non-UCSF location and will no longer be a UCSF employee, then please click on this link: to submit a proof of Professional Liability insurance coverage and claim history letter request. The COI and the letter are two very different things in the UC world unlike other institutions where the COI is one and the same for them. Please make sure you are requesting the correct one.
  2. UCSF COIs are provided through a company called Alliant (please click on the link for more details).
  3. Each UCSF department has a designated person(s) who requests COIs. Providers cannot request a COI; it can only be done by their department's COI Requestor(s). Please refer to A, B, or C below to find your department's COI Requestor(s).
A) Applying for a new Individual (Provider's) COI:
  1. First step for a provider to be given a COI is to have a contract or PSA (Provider Service Agreement) in place between UCSF and the non-UCSF institution that the provider will be working at. Without this a COI will not be issued. The provider needs to follow up with their department's contract team, ask for the person who handles CACTAS which can be found here by clicking on the link: CACTAS SupportThe PSA needs to be sent to the department's COI Requestor (see #4 below) to attach to the provider's COI application.
  2. Please make sure that that your department's COI Reviewer is aware of the requested COI and will approve it. If you do not know who your Reviewer is, please refer to #3 below.
  3. The process for obtaining a COI is done online with Alliant, a third party vendor. Here are the links to the UCSF/Alliant login pageand the UCSF Alliant COI Requestor Process how to guide for COI Requestors. The Alliant system will time out after seven minutes without warning and will not submit your COI request even though it still lets you press the submit button. The way you can be sure your submission went through is you’ll receive an automated email from noreply@alliant confirming your request. Do not leave the Alliant screen stating that your submission was successful until you receive the email.
  4. Not sure who your UCSF department's COI Requestor(s) and Reviewer(s) are? Here is the link to those contacts: UCSF Department COI Requestors & Reviewers. Please note that each UCSF department has a designated COI Requestor(s) and Reviewer(s). Healthcare providers cannot access or request a COI; it can only be done by their department's COI Requestor(s).
  5. After the COI Requestor successfully submits the COI request through the Alliant system, an automatic email notification will be sent to the COI Reviewer(s), who will then review it. If it is approved ("Reviewed" button in the Alliant system) by the COI Reviewer, it will then go to the Executive Director of Risk Management who will then give the final approval of the COI request. If the Executive Director does not approve it further information may be requested via email about the proposed activities listed on the COI application. It may be necessary to submit a copy of an underlying agreement / contract for review. If you are wondering whether or not your COI has been approved, please check with your department COI Reviewer first before coming to Risk Management to check on it. Majority of the time, it is waiting for their approval. The COI Requestor should receive an email notification that the COI Reviewer approved it.
  6. If the Executive Director of Risk Management approves the request, a COI will be issued. A copy of the COI will be sent to the three email addresses that the COI Requestor entered in on the Alliant platform (please see page 4 of the UCSF Alliant COI Requestor Process)
  7. COIs are good from July 1st through June 30th and need to be renewed annually, please see # B for more details. Ex: If you apply for a COI on June 25, 2023, it will expire on June 30, 2023.
B) How to renew a Provider's COI:
  1. The UCSF departments' COI Requestors manage the renewal requests for their providers. Every year the renewal process begins in mid-April and is finalized in early-June. The renewed COIs are then issued in mid-June by Alliant and sent by the COI Requestors directly to the non-UCSF facilities requiring them.   2. The COI renewal process is done directly via email and Box folders between the UCSF department COI Requestors, Alliant, and Risk Management.
  3. Renewed COIs are good from July 1st through June 30th and need to be renewed annually.
  4. What is the point of the COI Renewal spreadsheets: Alliant uses the spreadsheets to renew existing COIs. Once a department updates their spreadsheet and saves it in the assigned Box folder, Alliant works with that information to create the renewed COIs for the upcoming year.
  5. Please do not alter or rearrange your department's COI Renewal spreadsheet as Alliant uploads them to their system to be processed; it needs to be exact. ​​​Please only have your department's providers on the spreadsheet, do not combine with other departments' providers. Please do not remove columns. Please do not add in extra tabs. Please do not hide rows or columns. Please do not highlight rows or columns. Please do not remove the original filters. Please do not change column titles. The slightest change will delay the COI process, and you will be asked to change it back to its original format. C) How can I get a copy of a Provider's COI?
  1. The provider will be able to obtain a copy of their COI from their department's COI Requestor(s), please click on the link to find your COI Requestor(s): UCSF Department COI Requestors & Reviewers.
  2. The Requestor(s) will need to download the entire 'Renewed COI' Box folder in order to retrieve individual COIs that were completed during the COI renewal season in the springtime or do a search by the provider's full name in their Outlook inbox if the COI was requested through the Alliant portal.
  3. UCSF Health Risk Management does not provide copies of COIs.
D) What if I am a department COI Reviewer?
  1. Here is the link for the how to guide for Alliant UC Certificate Request Portal - COI Reviewer and the link for the UCSF/Alliant login page. *** In the COI Reviewer how to guide, follow the steps from page 1 through 9. Everything after that is for the UCSF COI Approver to follow. ***
  2. After you have clicked on the "Pending Review" link and then the "Review" button, as mentioned in the how to guide above, this will move it to the COI Approver as mentioned in letter E below.
E) How many Requestors, Reviewers, and Approvers are involved in the COI process? Who do I check with on the COI's approval status?

Before coming to Risk Management to check if your COI has been approved, please check with your department COI Reviewer first, click on this link: UCSF Department COI Requestors & Reviewers. There are three people involved in getting a provider's COI completed in the following order: the UCSF department COI Requestor, the UCSF department COI Reviewer, and finally the UCSF Health Risk Management Executive Director will approve it.

F) How do I opt out of COI email notifications since I am an Alternate COI Reviewer?

There are no distinctions in the system as to the Main or Alternate COI Reviewers for a department; both Reviewers will receive all notifications UNLESS they choose to “opt out” of receiving them. This is helpful when coverage is only needed due to vacations & leaves, as those Alternate COI Reviewers can then opt in and out as needed. Please follow these instructions on how to opt out.

  1. Log in to the Alliant portal using your UCSF login information: ucinsurancecert.alliant.com/landing.aspx
  2. On the home page there is a button in the upper right-hand corner titled “Administration”, click on it.
  3. That button will now change to “Options” with an arrow, click on it and select “Mail Settings
  4. Under the University of California, San Francisco Medical Center name are checkboxes, uncheck them to not receive email notifications.
